Booking & Cancellation Policy

Rejuvenate at Lake Wylie is committed to providing exceptional service. To ensure all clients receive timely attention and to respect the schedules of our service providers, we require the following policies for all appointments booked through our platform.

By booking an appointment, you acknowledge your understanding and agreement to the following terms and conditions.

1) Secure Booking & Deposit Requirements

All appointments require a valid credit card on file to be secured. The booking requirement depends on the type of service being scheduled:

Service Type Initial Booking Request Card Usage
Deposit Required Services (e.g., Dermal Fillers, Facials) A 50% Deposit of the service price is charged at the time of booking. This deposit is applied to the final cost of your service. Card used for deposit and for any applicable fees beyond the deposit amount.
Card-on-File Only Services (e.g., Prepaid Packages/Vouchers/Membership Services) No deposit charged at the time of booking. Card held only to enforce the Cancellation & No-Show Policy.

2) The 12-Hour Cancellation Deadline

We adhere to a 12-hour advance notice requirement to cancel or reschedule your appointment. This deadline allows us sufficient time to contact clients on our waiting list and fill the vacant time slot.

By booking an appointment, you acknowledge your understanding and agreement to the following terms and conditions.

Fee Enforcement

The following fees will be applied if the 12-hour deadline is missed:

Cancellation Scenario Definition Fee Applied
On-Time Cancellation Notification received 12 or more hours before the appointment. No Charge (Deposits are held as a credit on your client profile for future use or transferred to your new booking. Card refunds must be requested manually).
Late Cancellation Notification received less than 12 hours before the appointment. The initial deposit is forfeited, OR a fee up to 100% of the service price will be charged, subject to the service category below.
No-Show Failure to show up for your scheduled appointment without providing any notice. 100% of the service price.

Service-Specific Fee Structure

Service Category Late Cancellation/No-Show Fee Applied
Deposit Required Services The 50% deposit is Non-Refundable/Forfeited. For No-Shows, the remaining 50% may be charged, up to 100% of the service price.
Prepaid/Voucher Services A Flat Fee of $75 OR 50% price of session, at our discretion.

3) Late Arrival Policy

We value your appointment time. Please ensure you arrive at least ten minutes before your scheduled start time.

  • Appointments may need to be shortened to accommodate our schedule.

  • Late arrivals impacting the completion of your service are subject to being treated as a Late Cancellation or No-Show, and the full price (or package deduction/deposit forfeiture) may be applied.

4) Confirmation & Legal Disclaimer

As a courtesy, appointment confirmation messages are sent 48 hours in advance by both SMS and email to the contact information provided at the time of booking. Please respond to these confirmation messages to reserve your appointment.

  • Applicable taxes and fees will be retained and remitted.

  • All policies are subject to the contract and consumer laws of the state of South Carolina.

How We May Use and Share Your Information

We may use and share your health information for:

  • Treatment: Providing and coordinating your care.

  • Treatment Alternatives: Recommending options and services.

  • Care Involvement: Sharing with family/friends involved in your care (you can object).

  • Payment: Billing and collecting payment.

  • Healthcare Operations: Business activities like quality improvement.

  • Appointments: Reminders and scheduling.

  • Business Associates: Sharing with companies who help us operate (with confidentiality agreements).

  • Legal Requirements: Complying with laws.

  • Public Health: Reporting to authorities (e.g., disease control).

  • Abuse/Neglect/Domestic Violence: Reporting in these situations.

  • Healthcare Oversight: Sharing with licensing/oversight agencies.

  • Law Enforcement: Responding to legal orders.

  • Decedents: Sharing with coroners, funeral directors, etc.

  • Workers' Compensation: Sharing for related claims.

  • Serious Threats: Preventing harm.

  • Research: For approved studies.

  • Government Functions: For military/national security.

  • Legal Proceedings: For court cases.

  • Inmates: Sharing with correctional facilities.

Any other uses or disclosures require your written authorization. You can revoke this authorization at any time (except for disclosures already made).

Your Rights

You have the right to:

  • Request restrictions on information use/sharing (we must agree if you paid out-of-pocket).

  • Request confidential communications.

  • Access, inspect, and copy your records (fees may apply).

  • Request corrections to your records.

  • Receive a list of disclosures.

  • Receive a paper copy of this notice.

  • Be notified of breaches of unsecured information.

To exercise your rights or file a complaint, contact us or the federal government (Region IV, Office for Civil Rights). We will not retaliate against you for filing a complaint. We may change this notice; the revised notice will be posted and available upon request.

Digital & Website Privacy Addendum

Google Analytics and Tracking Technologies

Our website uses Google Analytics, a web analytics service provided by Google, Inc. ("Google"). Google Analytics uses cookies and similar tracking technologies to help us analyze how visitors use our site, measure marketing performance, and improve your overall user experience.

As part of this service, we have activated Google Signals. This feature allows Google Analytics to collect cross-device data about your traffic if you are signed into a Google account and have turned on "Ads Personalization." This data is entirely aggregated and anonymized; we never receive personally identifiable information or protected health information (PHI) about individual users through this feature, nor do we use it to identify any specific visitor.

How to Opt-Out

We respect your privacy and believe you should have control over your data. You can manage your tracking preferences and opt-out of Google’s data collection through the following methods:

  • Google Ads Settings: You can review and customize your Google advertisement preferences or turn off personalized ads entirely by visiting Google’s My Ad Center.

  • Google Analytics Opt-Out Browser Add-on: To prevent your data from being used by Google Analytics across all websites, you can download and install the official browser plugin available at the Google Analytics Opt-out Page.

  • Browser & Device Settings: You can also adjust your internet browser settings to reject all cookies or to notify you when a cookie is being set.
